Instructions for filling out the Utility Deposit Forms.

The Welcome Letter is information that you might find helpful as you move to a different community. The User Agreement will need to be filled out and dated and signed. The Deposit Sheet is information I need to set up your account. 

Please fill out all information that is in Blue. If you live outside of the city limits you have a choice if you want trash service or not.  If you live in the City Limits you are required to have trash service.  Return the forms to City Hall along with payment of $150.
